I declare that the information I have supplied is true and correct to the best of my knowledge. I understand that the support payment may be cancelled if it is determined that I was offered a support payment based on false or misleading information or documentation and that I may be required to repay monies received to which I am not entitled. I authorise Food and Beverage Development Fund SA Inc to take such steps as are necessary to confirm details provided in my application.

I agree to comply with the conditions of entry described previously and to any support payment conditions that may be determined by the Food and Beverage Development Fund SA Inc from time to time in relation to support payments. I recognise that failure to comply with any support payment conditions may result in the withdrawal of the support payment. Should the Food and Beverage Development Fund SA Inc withdraw any support payment, the Fund reserves the right to take such steps as are necessary to recover any outstanding funds or to withhold any funds not yet provided under the terms of the support payment.

I understand that the number and value of support payments to be made available in any year is determined by the Board and that all decisions relating to offers of support payments shall be final and no correspondence will be entered into. I give permission for the Food and Beverage Development Fund SA Inc to use non-confidential details from this application and relevant images for publicity purposes.

I understand that the support payments funding may or may not be taxable depending upon my individual circumstances and I will seek independent financial advice should I be successful in receiving a support payment offer.
